Expression of Interest: End of Life Care (EoLC)

The End of Life Care Integrator (EoLCI) team is seeking Expressions of Interest for adult EoLC projects.

They are keen to work with interested partners in Wales. They know that in Wales, as part of the Welsh Government’s ‘A Healthier Wales: A plan for Health and Social Care”, the ‘Palliative and End of Life Care Plan’ (extended to 2022), and Quality Statement update (2021) reducing variation and inequalities in access to health care and moving care closer to home are some of the key priorities that EoLC services can support.

They particularly encourage expressions of interest that focus on:

  • Increasing community-based EoLC services, actively supporting NHS recovery from the pandemic;
  • Addressing health inequalities in terms of access to end-of-life care and/or outcomes achieved;
  • Supporting innovation and news ways of working post-Covid such as embedding and sustaining learning from pilots’

What is the EOLCI?
The EOLCI was set up in 2016 following partnership working with organisations including the Department of Health and the NHS Confederation with support from Professor Bee Wee CBE. Starting in England, it tested whether an outcomes-based approach to EoLC could improve people’s quality of life in the last 12 months of life and increase the number of people who ‘die well’ by aligning system incentives.

The EoLCI Team has worked with systems to design, mobilise and deliver six EoLC projects.
Our projects have consistently met or exceed their target outcomes. Outcomes include people experiencing fewer days in hospital or achieving death in their preferred place.
