Salary: £21,166 - £22,911 per annum
Contract type: Fixed term: 1 year pilot; subject to ongoing funding
Pension: 8% non contributory
Hours of work: 37 hours per week
Holidays: 25 pa plus Bank Holidays
Office base: Bridgend County Borough with expected travel
Responsible to: Operations and Partnerships Manager, BAVO (or alternative identified Manager)

Who do we need?

We need a person to work as part of the Bridgend multi-agency safeguarding hub (MASH)/local community hubs Early Action teams to provide signposting and support to agency staff and individuals to access community based services.

We are looking for a person who can help individuals access the right help at the right time. Our Navigator role will likely know local communities very well and be aware of the wide range of services available within them. They are passionate about getting it right, able communicators and focused on using their experience to connect people with the help they need in their own communities.

Working daily with clients, health professionals, volunteers and service providers, our Community Navigator is focused on making sure a person’s journey to better wellbeing is as easy as possible - signposting and connecting them to existing services and opportunities. They will also work closely with the local authority, primary and secondary health care and police professionals, checking in on progress and making sure we can evidence the outcomes of the service are all important facets of the job – a job that makes a real difference in the lives of people.

This role is perfect for individuals with experience of working within a community team, setting  or health and/or wellbeing role using a case management approach. A practical understanding of primary and secondary care with the ability to champion and embrace change would be useful. The successful candidate will have a solution focused approach, be highly organised and be able to deliver to agreed service specifications and outcomes.
