How to better identify and support older carers in primary care settings?

Carers Trust Wales and Age Cymru are working together to deliver a three year national partnership project, funded by Welsh Government, to support the early identification of older carers and to make sure they get the information and support they need.

As part of this impactful project, Carers Trust Wales wants to hear from local organisations in the Cwm Taf Morgannwg area that deliver services for carers in primary care settings to support improved identification of older carers. As a result of this learning process, we will develop a series of resources which will have a direct and meaningful impact in older carers’ lives and their experience in primary care.

This session will be a unique opportunity for services, non-clinical healthcare professionals and local health boards to share their existing good practice and to co-produce useful and practical tools to increase rates of identification and support of older carers (those over the age of 60).
